This vital position uses a patient-centered and collaborative approach to optimize the medication record to improve patient outcomes, resolve discrepancies, and prevent medication errors.Health care providers and Pharmacy Technicians will partner with patients, families, and external resources to obtain accurate and complete medication histories   The Outpatient Pharmacy Technician II-Care Transitions obtains accurate medication histories from patients admitted to the hospital and assists pharmacist in reconciling home medications with those medications prescribed during their hospitalization. At discharge they assist and manage processes to provide medications to patients prior hospital discharge. Performs other pharmacy technician duties as required in the medication distribution processes to assure safe, efficient medication management. The Outpatient Pharmacy Technician II-Care Transitions performs these medication history capture and other tasks while under the supervision and instruction of a registered pharmacist.
