Join our dedicated team of Medication Reconciliation Pharmacy Technicians. This vital position uses a patient-centered and collaborative approach to optimize the medication record to improve patient outcomes, resolve discrepancies, and prevent medication errors.Health care providers and Pharmacy Technicians will partner with patients, families, and external resources to obtain accurate and complete medication histories


The Outpatient Pharmacy Technician II-Care Transitions obtains accurate medication histories from patients admitted to the hospital and assists pharmacist in reconciling home medications with those medications
prescribed during their hospitalization. At discharge they assist and manage processes to provide medications to patients prior hospital discharge. Performs other pharmacy technician duties as required in the medication
distribution processes to assure safe, efficient medication management. The Outpatient Pharmacy Technician II-Care Transitions performs these medication history capture and other tasks while under the supervision and instruction of a registered pharmacist.


Helps to coordinate with Pharmacy, Emergency Department and other hospital staff the medication management activities related to patient transitions of care from hospital admission to discharge
Serve as pharmacy technician resource for providing timely and accurate capture of pre-admission medication history upon admission, along with providing medication reconciliation as inpatient and at discharge from the hospital
Uses effective communication skills to interview patients, family, retail pharmacies and other care givers to compile the best possible medication history for the patient
Performs assigned daily Pharmacy Technician Care Transition activities
Perform other duties of the pharmacy technician related to medication procurement, preparation and distribution
Performs duties to ensure adherence to Department of Pharmacy regulatory and fiscal requirements


Well-developed interpersonal and communication skills with an orientation towards high customer service
Has basic computer skills
Ability to operate point of sale and cash management computer systems
Ability to prioritize and perform responsibilities accurately with attention to detail
Ability to handle frequent interruptions and adapt to changes in workload
Must possess strong oral and written communication skills


Licensure/Certification: AZ Pharmacy Technician Certification Licensure required within 90 days of hire date.*


Experience: Prior experience in a retail pharmacy


Note: *New employees will complete AZ Board of Pharmacy technician licensure within 90 days of hire, and
will be assigned appropriate tasks during licensure processing.
